Hydration Charting - Grafting:

Each day an aide writes down on a chart the fluid cc's consumed by a patient at each meal and snack. This is no longer necessary. Using the NurseWand scanner, a nurse aide scans the data into her computer system. The information is immediately available in chart and graph form.  Daily reports help you pick up errors and omissions. Surveyors, nurses and medical directors have accurate data which is actionable.  This system is the only bar code based, pro-active system to head off sentinel events such as bowel impactions and wounds.  Here are a couple of examples of the reports available:
